Cross-sectional Study on the Performance of Implants After More Than 10 Years in Function
Purpose:
This study assessed the prevalence of peri-implant diseases in patients with implant-supported prostheses functioning for at least 10 years in a private clinic, and identified potential risk and protective factors.
Materials And Methods:
Patients rehabilitated with dental implants whose final prostheses had been in function at least 10 years and had complete clinical and radiographic records were included. Clinical parameters assessed were plaque index (mpI), bleeding on probing (BOP), suppuration (SUPP), probing depth (PD), keratinized mucosa (KM), and peri-implant soft tissue dehiscence (PISTD), alongside radiographic marginal bone loss (MBL). Data on systemic health, implant, and prosthetic features were collected.
Results:
Thirty-five patients with 133 implants were analyzed (mean functional time 13.2 years). Implant survival was 95.5%. At the patient level, peri-implant mucositis and peri-implantitis were both 31.4%; at the implant level, 32.3% and 12.1%, espectively. Peri-implant diseases were associated with insufficient keratinized mucosa, high plaque index, smoking, bruxism, and osteoporosis. Prosthetic complications were mainly mild and did not compromise survival but were linked to greater bone loss, particularly in patients with osteoporosis. Bruxism, external hexagon connection, history of periodontitis, and immediate loading were associated with early mechanical complications.
Conclusion:
Long-term biological and mechanical complications are influenced by systemic conditions, implant characteristics, prosthetic design, and patient habits. These aspects should be integrated into treatment planning to reduce risks.
