Background:

  • Trabecular meshwork (TM) dysfunction is linked to elevated intraocular pressure (IOP) and primary open angle glaucoma (POAG).
  • Anterior segment optical coherence tomography (AS-OCT) offers high-resolution in vivo imaging of the TM.

Purpose of the Study:

  • To investigate in vivo morphological changes in the TM of POAG patients using AS-OCT.
  • To assess the diagnostic performance of AS-OCT for detecting POAG-related TM alterations.

Main Methods:

  • A prospective, observational, cross-sectional study included 154 patients (81 POAG, 73 controls).
  • AS-OCT was used to compare TM length, thickness, area, and reflectivity between POAG eyes and healthy controls.
  • Diagnostic performance was evaluated using receiver operating characteristic analysis.

Main Results:

  • POAG patients exhibited significantly lower TM thickness and area (P<0.001) and higher TM reflectivity (P=0.007) compared to controls.
  • AS-OCT demonstrated good diagnostic performance with an area under the curve of 0.81 for TM thickness.
  • No significant correlation was found between mean TM thickness and other parameters.

Conclusions:

  • AS-OCT reveals significant morphological TM changes consistent with degeneration in POAG.
  • AS-OCT imaging of the TM can complement retinal and optic nerve assessments for improved POAG screening and evaluation.
