[Laparoscopic IPOM+ versus IPOM for ventral hernia repair - are there clear advantages? A retrospective analysis]

Objective:
Optimal surgical treatment of ventral hernias is still a controversial issue. Hypothetically obvious advantage of IPOM+ is not always confirmed in practice.
Material And Methods:
A retrospective comparative analysis of treatment outcomes included 399 patients with ventral hernias who underwent laparoscopic intraperitoneal repair without suturing (IPOM) and with suturing of hernia defect (IPOM+) in 2020-2024. Of these, 331 (83%) patients underwent elective surgery, 68 (17%) ones - emergency surgery for hernia impingement. Outcomes were evaluated via analysis of outpatient database, EMC and registration of recurrences.
Results:
No significant differences in recurrence rate were obtained. There were 6 (2.26%) cases of paraprosthetic infection in the IPOM group and 6 (6.9%) cases in the IPOM+ group (p<0.05). Postoperative seromas occurred in 1 (1.15%) patient after IPOM+ repair and 12 (4.53%) patients after IPOM procedure. None patient had chronic pain syndrome. Mean surgery time was significantly shorter in the IPOM group.
Conclusion:
IPOM+ does not affect the incidence of recurrences and fluid accumulations increasing the risk of paraprosthetic infection.
