Objective:Episodic vestibular syndrome(EVS) represents a significant category of heterogeneous acute vertigo disorders. The etiology among many of these vertigo disorders has not to been elucidated yet, and there is no specific diagnostic biomarkers in clinical practice. The diagnosis approach for EVS primarily relies on clinical manifestations and differential diagnosis of the disorders. In the literature, several bedside examination batteries, such as HINTS, STANDING, and TiTrATE, as well as diagnostic paradigms have been proposed for differential diagnosis of vestibular vertigo, which have substantially advanced the clinical diagnostic capabilities of vestibular disorders for clinicians. Nevertheless, there is still a substantial rate of misdiagnosis for EVS in clinical practice. In recent years, the author has proposed the SCD Procedural Diagnostic Strategy and Pathway for vertigo disorders, as well as the SCD Procedural Diagnostic Strategy, Process, and Pathway for acute vestibular syndrome, respectively. Building upon these foundations and incorporating the clinical characteristics of episodic vestibular syndrome, this article proposes the SCD Diagnostic Strategy, Diagnostic Approach, and Differential Diagnostic Pathway for episodic vestibular syndrome.
