Background:

Purpose of the Study:

  • To report a rare case of prostatic adenocarcinoma metastasis to gynecomastia breast tissue.
  • To underscore the diagnostic challenges and imaging features of such rare metastases.

Main Methods:

  • A 72-year-old man with a history of prostate cancer underwent PSMA PET scan for rising PSA.
  • Imaging revealed an avid left breast lesion within gynecomastia tissue and ipsilateral axillary lymphadenopathy.
  • Histopathological confirmation via core biopsy and fine-needle aspiration.

Main Results:

  • Prostate-specific membrane antigen (PSMA) PET scan showed increased uptake in the left breast lesion and axillary lymph nodes.
  • Biopsy confirmed metastatic prostatic adenocarcinoma, not primary breast carcinoma.
  • The presentation mimicked primary breast malignancy.

Conclusions:

  • Prostate cancer metastasis to the breast is rare and can mimic primary breast cancer.
  • PSMA PET imaging can aid in detecting these metastases.
  • Histopathological confirmation is crucial for accurate diagnosis and management.
