Recurrent temporomandibular joint (TMJ) dislocation is an uncommon but disabling condition for which no universally accepted surgical solution exists. Existing procedures may be associated with morbidity, technical complexity, or unpredictable long-term outcomes. This Technical Note describes a novel ligamentoplasty technique for stabilization of the TMJ using a neoligament fabricated from ultra-high-molecular-weight polyethylene (UHMWPE), a material widely used in orthopaedic surgery. The technique involves placement of the neoligament through transosseous tunnels created in the mandibular condylar neck and the zygomatic arch via an endaural approach with Al-Kayat extension. Key technical steps, including tunnel positioning, neoligament tensioning, and fixation, are detailed. The surgical concept is illustrated using selected clinical cases in which the technique was applied following failure of previous treatments. In the authors' preliminary experience, the procedure was technically feasible and was associated with absence of further dislocation episodes during short-term follow-up. A limited decrease in mandibular range of motion was observed, without major perioperative complications. TMJ ligamentoplasty using UHMWPE neoligament represents a potential surgical option for selected patients with recurrent dislocation. As this report reflects an early technical experience, long-term safety, durability, and clinical effectiveness remain to be established through further study.
