Background:

  • Patient-derived organoids (LCOs) are increasingly used for drug screening.
  • Assessing their genomic fidelity and drug response prediction accuracy is crucial for clinical application.

Purpose of the Study:

  • To evaluate the accuracy of LCOs in recapitulating patient tumor genomic profiles.
  • To quantify the reliability of LCO-based drug screening.

Main Methods:

  • A systematic literature review was performed to pool existing evidence.
  • 20 patient-derived lung cancer organoids were established for internal validation.
  • Whole-exome and transcriptomic sequencing were utilized for genomic analysis.
  • Concordance in genetic profiles and drug sensitivity prediction accuracy were calculated.

Main Results:

  • Pooled concordance for mutational landscape between LCOs and tumors was 74%, and for gene expression was 70%.
  • LCOs maintained genetic fidelity irrespective of tumor histologic heterogeneity.
