T lymphocytes in anti-N-methyl-D-aspartate receptor encephalitis (NMDAR-E) patients show mitochondrial dysfunction and immune exhaustion. Specific mitochondrial markers in cerebrospinal fluid and blood may help distinguish NMDAR-E severity.
