Introduction:
Calcium phosphate precipitation in parenteral nutrition (PN) admixtures represents a significant clinical risk, with the potential to cause serious adverse events such as pulmonary embolism, catheter occlusion, or fatal outcomes. This review examines the physicochemical factors influencing calcium-phosphate compatibility, including the type and concentration of calcium and phosphate salts, final pH, temperature, sequence of component addition, and interactions with other elements such as lipids, glucose, and amino acids. The use of organic calcium and phosphate sources provides a broader compatibility margin and significantly reduces the risk of precipitation. Through a critical analysis of current evidence and published compatibility studies, this consensus outlines practical recommendations for safe PN formulation, including suggested maximum limits, technical precautions during preparation, and both visual and analytical control strategies. This document aims to serve as a guideline for healthcare professionals involved in the compounding and administration of PN, particularly in vulnerable populations such as neonates, where the risk of precipitation is markedly higher. The systematic implementation of these recommendations may significantly enhance the safety and efficacy of intravenous nutritional support.
