Central Giant Cell Granuloma: Diagnosis and Treatment in a Multidisciplinary Setting

A 7-year-old girl presented with painless swelling in the anterior region of the mandible as well as buccolingual expansion and spacing between the incisors. Imaging showed a well-defined, multilocular radiolucent lesion with cortical perforation, displacement of the canine tooth bud and root resorption of adjacent incisors. Diagnosis of central giant cell granuloma (CGCG) was confirmed by incisional biopsy and histopathology. Management involved a multidisciplinary team, including oral and maxillofacial surgery, pediatric dentistry, prosthodontics and orthodontics. Surgical enucleation with peripheral ostectomy was performed under general anesthesia, preserving the developing canine. Postoperative care included a fixed partial denture with a lingual bar to maintain space and support facial growth. Orthodontic intervention followed canine eruption to correct alignment. At 48-month follow-up, complete healing with no recurrence, normal facial growth and stable oral function were observed. This case highlights the importance of early multidisciplinary management for optimal pediatric CGCG outcomes.
