Pacemaker-mediated tachycardia (PMT) detection and termination vary by device, especially with prolonged ventriculoatrial conduction (VAC). Pacemakers with longer refractory periods and specific algorithms offer better protection against these arrhythmias.

- BIOTRONIK, Boston Scientific, and Medtronic adjusted atrial pacing after sensing within PVARP, unlike Abbott, which led to repetitive ventriculoatrial synchrony.

- Pacemaker anti-PMT performance differs significantly across manufacturers.
- Devices with longer PVARP and atrial pacing delay algorithms provide better protection against PMT during prolonged VAC.
- Individualized pacemaker selection and programming are crucial for managing pacemaker-mediated arrhythmias.
