Summary

The reverse Cotton osteotomy (RCO) effectively corrects flatfoot by plantarflexing the first ray, with optimal results seen using wedge sizes up to 3mm in cadaveric models. Surgeons should consider these findings for improved adult-acquired flatfoot correction.

Purpose of the Study:

  • To evaluate the effect of the reverse Cotton osteotomy (RCO) on first metatarsal position.
  • To provide surgical guidance for RCO in correcting adult-acquired flatfoot.
  • To determine optimal RCO wedge size for medial arch correction.

Main Methods:

  • Cadaveric study simulating weight-bearing conditions.
  • Radiographic measurement of Meary angle following 1- to 4-mm RCO.
  • Assessment of navicular-cuneiform incongruency angle (NCIA) and calcaneal pitch angle.
