Background:

  • Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL) is a rare cancer associated with textured breast implants.
  • The relationship between specific implant characteristics and BIA-ALCL development requires further investigation.

Purpose of the Study:

  • To investigate the association between breast implant characteristics (surface texture, volume, manufacturer) and replacement history with the risk of developing BIA-ALCL.
  • To provide evidence-based guidance for surgeons and patients regarding implant selection to minimize BIA-ALCL risk.

Main Methods:

  • A multicenter, retrospective case-control study involving 327 women without malignancy and 94 BIA-ALCL cases.
  • Analysis of surgery indication, implant manufacturer, volume, surface texture, indwelling time, and replacement history.
  • Conditional logistic regression was used to assess associations with BIA-ALCL.

Main Results:

  • All BIA-ALCL cases were associated with implants having roughness >10µm.
  • Increased implant volume correlated with a higher likelihood of BIA-ALCL (OR per 100cc = 1.81).
  • Breast reconstruction was linked to lower BIA-ALCL odds (OR = 0.53) compared to cosmetic augmentation; implant replacement also reduced risk (OR = 0.18).

Conclusions:

  • No association was found between BIA-ALCL and specific implant manufacturers or roughness <10µm.
  • Breast implant surface roughness should be a key consideration for surgeons and patients during treatment decisions.
  • These findings support informed choices for primary or secondary breast implant procedures to mitigate BIA-ALCL risk.
