Catatonia and mania: patterns of cerebral dysfunction.

Background:

Catatonia is a complex neuropsychiatric syndrome characterized by motor abnormalities.
Understanding the underlying factors of catatonia is crucial for diagnosis and treatment.
Previous research has not fully elucidated the distinct subtypes or contributing factors of catatonia.

Purpose of the Study:

To investigate the underlying structure of catatonic motor features using factor analysis.
To determine if distinct factors of catatonia correlate with specific diagnoses or patient characteristics.
To explore potential links between catatonia subtypes and treatment response.

Main Methods:

Factor analysis was applied to data from 55 hospitalized psychiatric patients exhibiting catatonic motor features.
Eight specific catatonic motor features were analyzed.
Statistical analysis was used to extract and interpret the resulting factors.

Main Results:

Two significant factors were identified, explaining 32% of the variance in catatonic symptoms.
Factor 1, characterized by mutism, negativism, and stupor, represented negativistic stupor and showed no correlation with diagnosis or treatment response.
Factor 2, including mutism, stereotypy, catalepsy, and automatic obedience, aligned with classical catatonia, was associated with mania, and tended to predict a positive treatment response.

Conclusions:

The findings suggest two distinct forms of cerebral dysfunction underlying catatonia.
Factor 2, associated with mania, may offer insights into the pathophysiology of manic episodes.
Further research with larger samples is warranted to validate these findings.
