Burn Injuries01:22

Burn Injuries

Burn injuries occur when the skin and underlying tissues are damaged due to exposure to heat, electricity, chemicals, radiation, or friction. They can vary in severity, from minor superficial burns to severe deep burns that can be life-threatening.
The damage results in the death of skin cells, which can lead to a massive loss of fluid. Dehydration, electrolyte imbalance, and renal and circulatory failure follow, which can be fatal. Burn patients are treated with intravenous fluids to offset...

Area of Science:

  • Plastic Surgery
  • Reconstructive Surgery
  • Biomaterials Engineering

Background:

  • Electrical facial burns are rare but severe injuries.
  • They often result in combined bone and soft-tissue defects.
  • Achieving single-stage reconstruction for these complex defects is challenging.

Purpose of the Study:

  • To evaluate a single-stage reconstruction technique for electrical burn-related facial defects.
  • To assess the efficacy of virtual surgical planning (VSP) and patient-specific implants (PSI) combined with a chimeric anterolateral thigh (ALT) flap.

Main Methods:

  • A 27-year-old male with a left hemifacial defect from an electrical burn underwent reconstruction.
  • Virtual surgical planning (VSP) was utilized to design a patient-specific polymethyl-methacrylate (PMMA) implant for skeletal restoration.
  • A chimeric anterolateral thigh (ALT) flap was employed for soft-tissue coverage.

Main Results:

  • The reconstruction achieved stable fixation of the patient-specific implant (PSI).
  • Complete survival of the anterolateral thigh (ALT) flap was observed.
  • Satisfactory malar projection was attained with no implant-related complications.

Conclusions:

  • Combining patient-specific implants (PSI) created with virtual surgical planning (VSP) and anterolateral thigh (ALT) flaps enables precise and reliable single-stage reconstruction.
  • This approach effectively addresses complex facial defects resulting from electrical burns.
