Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia01:16

Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Area of Science:

  • Interventional Radiology
  • Pain Management
  • Anesthesiology

Background:

  • Nerve blocks are crucial for opioid-sparing analgesia within multimodal pain management.
  • Interventional radiology (IR) increasingly requires proficiency in nerve block techniques.
  • Nerve blocks offer targeted pain relief, reducing opioid use, complications, and healthcare costs compared to general anesthesia.

Purpose of the Study:

  • To provide a comprehensive overview of nerve block mechanisms, indications, techniques, imaging guidance, and safety.
  • To emphasize the clinical applications of various nerve block categories in IR practice.
  • To review pharmacologic considerations, complications, and emerging applications of nerve blocks.

Main Methods:

  • Discussion of nerve block mechanisms, indications, and procedural techniques.
  • Review of imaging guidance, safety considerations, and pharmacologic aspects.
  • Analysis of complications, prevention strategies, and emerging applications in oncology and palliative care.

Main Results:

  • Nerve blocks encompass autonomic, mixed, peripheral somatic, fascial plane, and motor types.
  • Optimizing efficacy involves careful local anesthetic selection and adjunctive agents.
  • Strategies for preventing and managing complications like local anesthetic systemic toxicity are detailed.

Conclusions:

  • Interventional radiologists can enhance patient comfort, procedural efficiency, and outcomes by integrating nerve blocks.
  • Nerve blocks are foundational for IR in multimodal pain management.
  • Proper documentation, coding, and billing are essential for compliance and reimbursement.
