Summary

Black and African American men (BAAM) with prostate cancer (PCa) showed similar PSMA-PET tumor burden at recurrence compared to White American men (WAM), despite BAAM having more aggressive initial disease. This finding suggests comparable disease characteristics at recurrence between racial groups.

Background:

  • Prostate cancer (PCa) disproportionately affects Black and African American men (BAAM), with higher incidence, aggressiveness, and recurrence risk post-radical prostatectomy (RP).
  • Biochemical recurrence (BCR) detection at low prostate-specific antigen (PSA) levels (< 2.0 ng/mL) is crucial for timely intervention.

Purpose of the Study:

  • To investigate racial differences in 68Ga-PSMA-11 PET/CT detected biochemical recurrence (BCR) patterns in Black and African American men (BAAM) versus White American men (WAM) post-radical prostatectomy (RP).
  • To compare 68Ga-PSMA-11 PET/CT imaging parameters between BAAM and WAM at low PSA levels post-RP.

Main Methods:

  • Retrospective analysis of 49 men (17 BAAM, 32 WAM) with BCR post-RP who underwent 68Ga-PSMA-11 PET/CT.
  • Collected and compared clinical data (PSA, PSA density) and 68Ga-PSMA-11 PET/CT parameters (detection rate, SUVmax, SUVmean, PSMA tumor volume [PSMA-TV], total lesion PSMA [TL-PSMA]) between racial groups.

Main Results:

  • Overall detection rate was 57% (47% in BAAM vs. 63% in WAM; p=0.39).
  • BAAM had significantly higher baseline PSA and PSA density (p<0.001).
  • Despite higher PSMA-TV and TL-PSMA in BAAM, tumor burden parameters were comparable between groups at recurrence, with no statistical significance.

Conclusions:

  • 68Ga-PSMA-11 PET/CT revealed comparable PSMA-derived tumor burden in BAAM and WAM at biochemical recurrence, despite BAAM's more aggressive initial diagnosis.
  • Findings suggest similar disease characteristics at recurrence, warranting further investigation in diverse populations.
