This study aimed to map the bibliometric characteristics, research trends, and thematic evolution of randomized controlled trials conducted in the field of pediatric nursing.

A descriptive bibliometric design was employed. Data were retrieved from the Web of Science Core Collection to examine publication patterns, influential journals, highly cited studies, and emerging research themes. Bibliometric analyses and visualizations were conducted using the Bibliometrix package in R software.

A total of 133 randomized controlled trials published between 2021 and 2025 were included, involving 418 authors across six journals. Journal of Pediatric Nursing emerged as the most influential source, accounting for the majority of publications (n = 116). Author keyword analysis revealed a strong thematic focus on pain, anxiety, fear, children, and distraction, indicating an emphasis on symptom management and children's hospital experiences. Trend and thematic analyses highlighted both well-established research areas and emerging topics within pediatric nursing randomized controlled trials.

The findings demonstrate a substantial growth in randomized controlled trials in pediatric nursing in recent years, reflecting the discipline's increasing commitment to evidence-based practice. The identified thematic concentrations and research gaps provide a structured overview of current priorities and point to underexplored areas requiring further investigation.

Mapping research trends in pediatric nursing randomized controlled trials may support nurses, educators, and researchers in strengthening evidence-based interventions and guiding future experimental research agendas.
