In the presence of dual retrograde pathways, conventional Para-Hisian pacing may fail to reveal the slow pathway, likely due to masking by fast retrograde conduction. In such cases, Para-Hisian extra pacing-combining programmed right ventricular stimulation with differential His bundle capture-enables assessment of the slow retrograde conduction.
