Summary

Parvovirus B19 infection can mimic rheumatoid arthritis (RA) flares, causing diagnostic challenges. Prompt diagnosis and treatment with intravenous immunoglobulin (IVIG) led to recovery in a patient with RA and pure red cell aplasia (PRCA).

Background:

Parvovirus B19 infection can present with symptoms mimicking autoimmune disease flares, such as rheumatoid arthritis (RA).
Diagnostic challenges arise when parvovirus B19 mimics rheumatologic conditions, potentially leading to misdiagnosis.

Purpose of the Study:

To report a case of parvovirus B19 infection mimicking a rheumatoid arthritis flare.
To highlight the importance of considering parvovirus B19 in patients with atypical rheumatologic and hematologic presentations.

Main Methods:

A case report of a 50-year-old man with seropositive RA presenting with polyarthropathy and anemia.
Diagnostic workup included bone marrow aspiration and PCR for Parvovirus B19 DNA.
Treatment involved intravenous immunoglobulin (IVIG).

Main Results:

The patient presented with symptoms initially managed as a rheumatoid flare but revealed severe anemia and reticulocytopenia.
Bone marrow examination showed pure red cell aplasia (PRCA) with characteristic findings.
Parvovirus B19 DNA was detected, and IVIG treatment resulted in rapid hematologic recovery.

Conclusions:

Parvovirus B19 infection should be considered in the differential diagnosis of patients with rheumatologic or hematologic abnormalities, especially in immunocompromised or chronically ill individuals.
Early recognition and treatment of parvovirus B19 are crucial for preventing misdiagnosis and ensuring favorable patient outcomes.
