Health Harbor Hamburg (H3): A Federated Health Data Space

Background:
Established health IT systems and their ancillary systems are able to manage sensitive health data, but are unable to function as a fully-fledged health data space (HDS) either as individual system or in combination. National health information exchange (HIE) platforms aggregate data controlled by the patient but do not constitute an HDS. As a result, the establishment of HIE, in particular featuring a HDS functionality, has proven to be difficult.
Objectives:
The objective is to present the approach taken for a federated, regional HDS by the Health Harbor Hamburg (H3) initiative.
Methods:
A user-centered development using a co-creation process involving medical, organizational and data protection stakeholders led to requirements for design of the H3 platform as HDS architecture using established interoperability standards.
Results:
The established H3 platform provides a secure HDS. Patient rights were reflected in a case-based consent model. For integrating the H3 participants IT infrastructures with the HDS a H3 Gateway and a H3 Adapter were realized.
Conclusion:
The platform is currently in pilot operation with 20 institutions and has gained positive feedback from clinical users.
