Summary

Clear aligner bite planes do not improve anterior intrusion but effectively prevent spontaneous molar intrusion. This helps maintain vertical molar position during deep overbite correction.

Background:

  • Deep overbite correction using clear aligners presents challenges, including reduced anterior intrusion and unintended molar intrusion.
  • Bite planes are sometimes used to mitigate these issues, but their efficacy is debated.

Purpose of the Study:

  • To evaluate the impact of bite planes on anterior intrusion efficiency and posterior tooth displacement during clear aligner treatment for deep overbite.

Main Methods:

  • A retrospective study analyzed 36 adult patients with deep overbite treated with clear aligners (Smartee® system).
  • Patients were divided into a bite plane group (n=15) and a control group (n=21).
  • Digital models were used to measure tooth displacement and calculate anterior intrusion accuracy.

Main Results:

  • No significant difference in anterior intrusion accuracy was observed between the bite plane and control groups.
  • The control group showed spontaneous molar intrusion (0.1-0.2 mm), correlated with treatment duration.
  • The bite plane group exhibited no spontaneous molar intrusion, with slight extrusion instead, and no correlation with treatment duration.

Conclusions:

  • Bite planes in clear aligners do not enhance anterior intrusion or affect labiolingual tooth movement.
  • They effectively prevent spontaneous molar intrusion, maintaining vertical molar position during deep overbite correction.
  • Bite planes are recommended for deep bite correction in normodivergent and hypodivergent patients.
