Study Designs in Epidemiology

Epidemiological study designs are fundamental tools for investigating the distribution, determinants, and control of health conditions in populations. They help researchers understand the relationships between exposures and outcomes, and they broadly fall into two categories: "observational" and "experimental" studies.
Observational studies are those where the researcher does not intervene but rather observes natural variations. They include cross-sectional, cohort, and case-control studies.

This study evaluates Sharing Health Education Resources (SHER), a multimedia messaging service (MMS) intervention, to increase HIV testing rates among sexual and gender minority (SGM) adolescents. Findings will inform strategies to improve engagement in sexual health care for this population.

Area of Science:

  • Public Health
  • Digital Health Interventions
  • Adolescent Health

Background:

  • Sexual and gender minority (SGM) adolescents exhibit suboptimal HIV testing rates despite risk factors.
  • Effective interventions for increasing HIV testing among SGM adolescents are scarce.
  • Reaching SGM adolescents for digital health interventions outside research settings presents implementation challenges.

Purpose of the Study:

  • To adapt a digital sexual health intervention for inclusivity of all SGM adolescents and incorporate advancements in HIV prevention.
  • To evaluate the effectiveness of the adapted multimedia messaging service (MMS) intervention (SHER) on HIV testing rates in SGM adolescents.
  • To identify best practices for reaching and engaging SGM adolescents in digital health interventions in real-world settings.

The Participant-Reported Implementation Update and Score (PRIUS): A Novel Method for Capturing Implementation-Related Data Over Time

Published on: February 19, 2021

  • Hybrid type 1 effectiveness-implementation randomized controlled trial (RCT) involving 360 US SGM adolescents aged 13-19.
  • Intervention adapted based on information-motivation-behavioral skills theory, stakeholder feedback, and modern technology.
  • Comparison of SHER intervention against an information-only control condition for primary outcome of HIV testing.

Main Results:

  • Data collection for intervention updates concluded January 2025; publication planned Fall 2026.
  • As of April 2026, 295 participants enrolled in RCT; full data collection and analysis completion planned Winter 2027.
  • Implementation data collection completed September 2025; publication of best practices planned Summer 2026.

Conclusions:

  • The study will determine if an MMS intervention (SHER) increases HIV testing among SGM adolescents compared to an information-only control.
  • Expert and adolescent insights will yield best practices for engaging SGM youth in sexual health interventions.
  • This research aims to enhance sexual healthcare engagement and HIV testing among at-risk SGM adolescents.
