Abstract:
In response to the increasing pressure on healthcare systems, medical homes offer a promising solution. The 1st French-Speaking Swiss Forum on Medical Homes provided cross-sectoral, local, and international perspectives. Medical homes are characterized by multiprofessional and colocated primary care teams, a territorial grounding, and a holistic individual approach, while also addressing population needs. In Switzerland, several innovative projects exist, yet few explicitly identify as medical homes. The cantons of Geneva and Vaud represent exceptions, with public funding supporting coordination and prevention activities. International evidence demonstrates that the impact of medical homes depends on political and financial frameworks. Originating from equity-focused movements, they improve healthcare access, but their transformative potential remains limited without adequate funding and shared governance structures.
