Pulsed field ablation (PFA) effectively isolates pulmonary veins for atrial fibrillation treatment. This study shows PFA achieves acute and durable pulmonary vein isolation with a strong safety profile in preclinical models.

Area of Science:

  • Cardiology
  • Electrophysiology
  • Medical Devices

Background:

  • Pulsed field ablation (PFA) utilizes nonthermal irreversible electroporation for targeted cardiomyocyte ablation.
  • PFA is an emerging therapeutic modality for treating atrial fibrillation.

Purpose of the Study:

  • To evaluate the safety and efficacy of a novel PFA system for pulmonary vein isolation (PVI).
  • To assess acute and durable PVI achievement using a PFA system integrated with 3D mapping.

Main Methods:

  • A preclinical study in six swine using a PFA system with flexible circular catheters and a 3D mapping system (3DMS).
  • Bipolar, biphasic waveforms were delivered for PVI at the right superior pulmonary vein (RSPV) and common inferior pulmonary vein (CIPV).
  • Electrophysiological mapping, gross necropsy, and histology were performed immediately post-procedure and at 3-month follow-up.

Published on: October 28, 2020

Main Results:

  • All targeted pulmonary veins were successfully isolated, achieving acute and durable PVI at 3-month follow-up in all animals.
  • Histopathology confirmed 100% transmural fibrotic lesions with no inflammation or thrombus.
  • No ventricular arrhythmias or periprocedural adverse events, including collateral damage (esophageal injury, phrenic nerve paralysis), were observed.

Conclusions:

  • The 3DMS-integrated PFA system demonstrated high acute and chronic efficacy for PVI.
  • Durable lesion formation and a favorable safety profile, free from collateral tissue injury, were confirmed.
  • This preclinical study supports the potential of PFA as a safe and effective treatment for PVI.
