How I Do It: "And That's a BINGO!" Using a Self-Directed, Gamified Instrument to Structure Learning on the Obstetrics

Objective:
Clinical exposure on undergraduate obstetrics clerkships is highly variable, leaving medical students uncertain about expectations and inconsistent in achieving required clinical and procedural skills. We developed and implemented an obstetrics (OB) BINGO board to clarify learning objectives, promote learner autonomy, and active engagement in a dynamic clinical environment.
Intervention:
An OB BINGO board, grounded in self-directed learning theory and principles of gamification, was developed by mapping Association of Professors of Gynecology and Obstetrics clerkship learning objectives to 24 observable and feasible clinical and procedural tasks. The instrument was iteratively refined using a clerkship-cycle rapid evaluation approach.
Setting:
The intervention was implemented at a tertiary academic medical center on the labor and delivery unit.
Participants:
Clinical medical students rotating on the obstetrics and gynecology clerkship. A total of 118 students participated across 12 clerkship cycles.
Results:
Among participating students, 88.5% achieved at least one BINGO, reflecting high engagement with the instrument. The OB BINGO board translated clerkship learning objectives into clearly defined, achievable, and publicly celebrated clinical and procedural goals. Rapid-cycle refinement ensured goals remained achievable within the clinical workflow. The board supported goal setting, clarified expectations, and encouraged active participation in clinical activities. Learners reported that the BINGO board clarified expectations, promoted proactive engagement, and provided a structured framework for identifying clinical learning opportunities.
Conclusion:
A gamified BINGO board offers a practical, scalable approach to structuring learning in clerkships with variable clinical exposure. This How I Do It describes a reproducible framework that can be adapted by other clinical educators without adding substantial burden to learners or supervisors.
