The proper selection of venous devices is essential in infusion therapy, and the use of a flowchart to select the appropriate catheter type supports better therapy outcomes and prevents adverse events in patients.

The aim of this project was to improve selection of the appropriate venous device by nurses through the implementation of best practices in a tertiary hospital in São Paulo, Brazil.

This best practice implementation project followed the seven-phase JBI evidence implementation methodology, which is centered on audit, feedback, and the management of barriers and facilitators. The project was carried out in two hospital units, one for frail and elderly inpatients and the other for semi-intensive care patients.

Eight audit criteria were used. An improvement was observed in the nursing team's knowledge of the availability and use of the flowchart for selecting the appropriate venous catheter, which increased from 88.9% to 100%. There was also a 54.2% reduction in cases involving multiple peripheral puncture attempts. Among the 40 medical records reviewed, 70% of the patients who required intravenous therapy for more than 5 days in the follow-up audit received the appropriate vascular device.

There was an improvement in the team's knowledge, use of the flowchart, and appropriate selection of venous devices, especially for prolonged therapies. The audits reinforced evidence-based practice, promoted more accurate records, and helped maintain quality of care.
