Background:

Weight recurrence (WR) is a frequent long-term complication following bariatric surgery, diminishing procedure efficacy and obesity-related comorbidity resolution.
The reported prevalence of WR varies significantly, influenced by differing definitions and follow-up durations.

Purpose of the Study:

To systematically review current definitions, contributing factors, and management strategies for WR post-bariatric surgery.
To highlight the critical role of early detection and comprehensive follow-up in managing WR.

Main Methods:

A narrative review of existing literature on WR after bariatric surgery was performed.
The review focused on epidemiology, pathophysiology, risk factors, and therapeutic interventions, encompassing behavioral, pharmacological, endoscopic, and surgical approaches.

Main Results:

WR can affect over 70% of patients long-term, contingent on the criteria used.
Primary contributors include behavioral factors, poor adherence to nutritional guidelines, hormonal shifts, anatomical alterations, and inadequate postoperative care.
Early detection within two years post-surgery enhances intervention success rates.
Treatment spans lifestyle changes, psychological support, and advanced endoscopic/surgical methods, with variable outcomes.

Conclusions:

WR is a complex, multifactorial condition necessitating personalized, multidisciplinary management.
