Purpose of the Study:

  • To compare surgical outcomes of robotic MVr in patients with and without thoracic deformities.
  • To assess the impact of thoracic deformity on operative times during robotic MVr.
  • To evaluate the safety and efficacy of robotic MVr in a cohort with pectus excavatum.

Main Methods:

  • A retrospective review of 359 patients undergoing robotic MVr for degenerative mitral regurgitation between 2018-2022.
  • Patients were categorized into a deformity group (Haller index ≥3.25, n=35) and a non-deformity group (Haller index <3.25, n=324).
  • Surgical outcomes, operative times, and early postoperative results were compared between the two groups.

Main Results:

  • No significant differences were observed in operative time (80±29 vs. 80±30 min) or aortic cross-clamp time between the deformity and non-deformity groups.
  • Early outcomes, including ICU stay (1.92±1.59 vs. 1.77±1.26 days), hospital stay, mortality, stroke, and re-exploration, were comparable.
  • One-year outcomes for mitral regurgitation grade, survival, and freedom from reoperation were similar between groups.

Conclusions:

  • Robotic mitral valve repair is a viable option for patients with thoracic deformities.
  • Thoracic deformity does not appear to negatively impact operative time or early outcomes in robotic MVr.
  • Patients with pectus excavatum undergoing robotic MVr experience comparable results to those without deformities.
