Comparative Evaluation of Remineralizing Agents on Hypersensitivity Scores in Molar Incisor

Objective:
This randomized clinical trial aimed to evaluate and compare the efficacy of four desensitizing agents-fluoride varnish (Duraphat), FluoriMax varnish (Elevate), surface prereacted glass-ionomer (S-PRG) filler (Shofu), and silver diamine fluoride (SDF)-in managing hypersensitivity in molar incisor hypomineralization (MIH)-affected teeth.
Materials And Methods:
A total of 80 children aged between 6 and 12 years, each presenting with at least one incisor and one first permanent molar showing yellow or brown MIH opacities, were selected for the study. The participants were randomly divided into four treatment groups-Duraphat, FluoriMax, S-PRG, and SDF. Hypersensitivity levels were assessed using the Schiff Cold Air Sensitivity Scale (SCASS) at baseline and then reevaluated at 1, 3, and 6 months following treatment. Intergroup differences were analyzed using ANOVA, followed by Tukey's honestly significant difference (HSD) post hoc test.
Results:
All groups demonstrated a significant reduction in mean SCASS scores over the 6-month period (p < 0.01). The greatest reduction was seen in the SDF group (mean change: - 1.36), followed by S-PRG (- 1.24), FluoriMax (- 1.04), and Duraphat (- 0.74).
Conclusion:
SDF demonstrated the greatest reduction in hypersensitivity in teeth affected by MIH. This was followed by S-PRG and FluoriMax, while fluoride varnish (Duraphat) was the least effective among all agents tested.
Clinical Significance:
This study highlights the application of newer remineralizing agents, particularly SDF and S-PRG, in relieving hypersensitivity associated with MIH in children, thereby augmenting patient comfort and promoting the longevity of permanent dentition.
