EURADOS survey on IGRT practices in European radiotherapy clinics

Introduction:
This survey was part of the EURADOS project to investigate imaging practices and estimate patient doses from IGRT. This study evaluated IGRT practices in European radiotherapy clinics, including quality assurance and dose-related issues of IGRT, as well as machine- and site-specific parameters.
Methods:
The questionnaire was divided into two sections: the first section included 12 questions related to general information, while the second section included 5 questions related to machine- and site-specific issues in IGRT practice. The questionnaire was distributed to the radiotherapy centres through EURADOS contacts and medical physics societies. Survey data were collected in 2023-2024.
Results:
In total, 37 radiotherapy centres from 14 countries participated in the survey. KV CBCT was the most used IGRT modality. Quality assurance of imaging systems was performed in 29 clinics. KV CBCT imaging was used in each fraction in most clinics. Several clinics had optimized kV CBCT protocols and dose recording. Volume imaging is mostly used for head, pelvis, and prostate regions, while planar imaging is used for thorax, breast, and lung regions. Results showed broad ranges for all imaging parameters reported in the study.
Conclusion:
The survey revealed wide variation in imaging parameters and dose-recording practices, as well as the absence of national protocols in most countries. Despite the long history of IGRT, the lack of reporting on radiation doses obtained from imaging and (non-)optimized protocols highlight the urgent need for coordinated efforts by European expert bodies and regulators to develop standardized guidelines.
