Introduction:
Sepsis is a dysregulated response to infection that can lead to organ dysfunction and requires urgent management. Advanced respiratory support (high-flow oxygen therapy and/or non-invasive mechanical ventilation) has emerged as a therapeutic option in patients with respiratory failure.
Objectives:
To identify factors associated with therapeutic failure (defined as the need for invasive mechanical ventilation or death) in patients with respiratory sepsis treated in a respiratory intermediate care unit. A retrospective observational study including 47 patients admitted between January 2021 and December 2023 was conducted. Therapeutic failure was associated with elevated procalcitonin levels (p = 0.046). Lower PaO2/FiO2 ratios and the use of vasopressors were also associated with increased mortality (p = 0.007).
Conclusion:
Advanced respiratory support could be considered a treatment option in this context. Elevated procalcitonin, hypoxemia at admission, and the need for vasopressors were associated with worse outcomes, although these findings should be interpreted with caution.
