Imaging Studies for Cardiovascular System VI: Calcium -Scoring CT

Calcium-Scoring CT ScanA calcium-scoring CT scan, also known as coronary artery calcium (CAC) scan, detects calcium deposits in the coronary arteries. This test assesses the risk of coronary artery disease (CAD), which can lead to cardiovascular events such as angina, heart failure, and sudden cardiac arrest.A calcium-scoring CT scan is generally recommended for individuals at intermediate risk of CAD without symptoms. It includes:Men aged 40-75 and women aged 50-75: Especially those with a...

Area of Science:

  • Interventional Radiology
  • Hepatobiliary Oncology
  • Medical Imaging Analysis

Background:

  • Hepatocellular carcinoma (HCC) is a major global health concern.
  • Drug-eluting bead transarterial chemoembolization (DEB-TACE) is a standard treatment for HCC.
  • Predicting treatment response in real-time is crucial for optimizing patient management.

Purpose of the Study:

  • To develop and validate a simple intraoperative scoring system for predicting objective response (OR) by modified Response Evaluation Criteria in Solid Tumors (mRECIST) after DEB-TACE.
  • To utilize preoperative and cone-beam CT (CBCT) features for real-time prediction.

Main Methods:

  • Retrospective analysis of 274 HCC patients (386 tumors) undergoing first-session DEB-TACE.
  • CBCT assessment of marginal contrast saturation (MCS), capsule presence, and vascular lake phenomenon (VLP).
  • Development and validation of a point-based scoring system using multivariable logistic regression and AUROC analysis.

Main Results:

  • The objective response rate (ORR) was 67.9%.
  • Tumor diameter, capsule presence, VLP, and MCS grade were independent predictors of OR.
  • The developed 1-10-point scoring system demonstrated excellent discrimination (AUROC 0.93) in both development and validation sets.

Conclusions:

  • An intraoperative, imaging-only scoring system accurately predicts short-term mRECIST response after DEB-TACE.
  • The system incorporates tumor size, capsule, VLP, and MCS.
  • This tool facilitates rapid decision-making and improves subsequent patient management.
