Background:

  • Impulsivity is frequently observed in eating disorders (EDs) with comorbid self-harm.
  • Previous reviews have not systematically examined impulsivity-related traits in ED groups with co-occurring self-harm.

Purpose of the Study:

  • To systematically review and meta-analyze differences in impulsivity-related traits between ED groups with and without self-harm or suicidal behavior.
  • To identify specific impulsivity domains associated with self-harm in ED populations.

Main Methods:

  • Systematic literature search following PRISMA 2020 guidelines, including electronic databases and author requests.
  • Inclusion of comparative observational studies from 1994 onwards comparing ED groups with vs. without self-harm/suicidal behavior on impulsivity measures.
  • Narrative synthesis and random-effects meta-analyses for each impulsivity domain, with risk of bias assessment.

Main Results:

  • ED groups with self-harm/suicidal behavior showed significantly higher scores in Negative Urgency (g=0.55), Lack of Premeditation (g=0.34), Lack of Perseverance (g=0.20), and Sensation Seeking (g=0.10).
  • No significant differences were found for Positive Urgency, based on limited data.

Conclusions:

  • Negative Urgency is the primary impulsivity trait linked to self-harm in ED groups, supporting its role in emotion-driven impulsivity.
  • Findings should be interpreted cautiously due to the small number of studies, heterogeneity, and predominantly female samples.
