Abstract:
In this 12-month, virtual, RCT, we compared usual care to a nurse-led primary palliative care intervention on chronically ill, community-dwelling older adults across the United States with varying levels of cognitive impairment. Significantly higher rates of advance care planning (89.1% vs 3.0%) and reporting of supportive care needs (86.1% vs 29.3%) in the intervention vs usual care arm highlights the improvement in the care of older adults with cognitive impairment.
