Area of Science:

  • Neonatology
  • Pediatric Cardiology
  • Infectious Diseases

Background:

  • Neonatal sepsis is a leading cause of infant mortality.
  • Septic shock in neonates causes impaired tissue perfusion and organ dysfunction.
  • Hemodynamic alterations in neonatal sepsis are often under-recognized.

Purpose of the Study:

  • To characterize the hemodynamic profile of neonates with sepsis using functional echocardiography.
  • To enable early detection of circulatory dysfunction in septic neonates.
  • To evaluate the need for vasoactive support and its impact on outcomes.

Main Methods:

  • Prospective observational study of 288 neonates with clinical sepsis.
  • Sepsis diagnosis based on clinical signs and organ dysfunction assessed by nSOFA score.
  • Hemodynamic assessment via functional echocardiography evaluating preload, contractility, and afterload.

Main Results:

  • 35.4% of septic neonates developed septic shock, common in preterm/low-birth-weight infants.
  • Shock group showed impaired myocardial contractility and altered perfusion.
  • Mortality was significantly higher in neonates with septic shock (46.1% vs. 9.1%).

Conclusions:

  • Septic shock in neonates is associated with significant hemodynamic compromise and high mortality.
  • Functional echocardiography aids early recognition of circulatory dysfunction.
  • Timely hemodynamic interventions can potentially improve neonatal outcomes.
