We report a case of Langerhans cell histiocytosis (LCH) in a 2-year-old boy who presented with scalp swelling. Imaging work-up revealed dural involvement, which is a rare manifestation in LCH. Dural LCH can present with imaging features that closely resemble meningioma or inflammatory pachymeningitis. Combined MRI and FDG PET imaging provides complementary information. MRI explains the structural and anatomic details of dural involvement, and PET aids in evaluating metabolic activity, facilitating early recognition, accurate staging, and treatment response assessment in CNS LCH.
