When Size Matters: Advanced Treatment of Acute Pulmonary Embolism Caused by a Large Snake-Like Thrombus

A 56-year-old female patient presented with pulmonary embolism-related obstructive shock complicated by cardiac arrest. Computed tomography revealed a large snake-like thrombus. After unsuccessful imaging-guided transcatheter aspiration, targeting one end of the thrombus, surgical embolectomy was effective in removing a 48 cm single thrombus. A thorough evaluation of pre-procedural images could help determine the best treatment strategy and should be an essential part of the multidisciplinary decision-making process.
