Survival outcomes for children with congenital heart disease (CHD) have improved substantially because of advances in surgery, interventional cardiology, imaging, intensive care, and perioperative management. As congenital cardiology increasingly evolves into a lifespan discipline, many contemporary challenges extend beyond procedural success alone. Children and families frequently navigate neurodevelopmental vulnerability, developmental care needs, psychosocial stress, educational disruption, healthcare transitions, and long-term adaptation across developmental stages. Outcomes are also influenced by social determinants of health, family resources, healthcare access, and organizational environments. Developmental Systems Cardiology (DSC) is proposed as an integrative framework that conceptualizes congenital heart disease as a lifelong adaptive process shaped by biological, developmental, psychosocial, community, and healthcare influences. The framework highlights how organizational factors, implementation processes, and health inequities contribute to long-term outcomes. This article explores how Developmental Systems Cardiology may inform neurodevelopmental surveillance and care, family-centred communication, psychosocial support, transition medicine, organizational culture, clinician wellbeing, implementation science, professional formation, and integrated lifespan care. Particular emphasis is placed on individuals with complex congenital heart disease, including Fontan circulation, who require coordinated longitudinal care across multiple systems. Developmental Systems Cardiology proposes that future progress in congenital heart disease will depend not only on improving survival and physiological outcomes, but also on strengthening the developmental, relational, educational, community, and organizational systems that support children and families throughout life. Within this framework, adaptation becomes a central clinical outcome, implementation target, and organizing principle across the lifespan.
