Pulmonary leukemic infiltration (PLI) is a rare complication of acute myeloid leukemia often misdiagnosed as pneumonia. This case highlights a unique presentation of PLI in a relapsed patient without hyperleukocytosis, necessitating a high index of suspicion. Aggressive salvage chemotherapy followed by stem cell transplantation was effective in resolving the infiltration and achieving remission.
