Agreement between pentacam AXL and IOLMaster 700 axial length measurements in a routine cataract population

Purpose:
To evaluate the agreement between axial length (AL) measurements obtained using Pentacam AXL and IOLMaster 700 in a routine cataract population and to assess the potential influence of endothelial and optical quality parameters on inter-device differences.
Methods:
This retrospective observational study included 65 eyes of 65 patients. AL measurements were obtained using Pentacam AXL and IOLMaster 700 during the same visit. Agreement was assessed using Pearson correlation, intraclass correlation coefficient (ICC), and Bland-Altman analysis. Associations between inter-device AL differences (ΔAL) and endothelial and optical parameters were analyzed.
Results:
The mean axial length was 23.46 ± 0.95 mm with IOLMaster 700 and 23.44 ± 0.94 mm with Pentacam AXL. A very strong correlation was observed between devices (ICC = 0.998; 95% CI: 0.996-0.999, r = 0.998, p < 0.001). The mean ΔAL was 0.019 ± 0.054 mm, with 95% limits of agreement ranging from -0.087 mm to +0.124 mm. No significant associations were identified between ΔAL and endothelial or optical quality parameters.
Conclusions:
Pentacam AXL and IOLMaster 700 demonstrated excellent agreement in AL measurements in a routine cataract population. These findings support the clinical reliability of Pentacam AXL for routine AL assessment in cataract surgery planning.
