Toxoplasmosis01:28

Toxoplasmosis

Toxoplasmosis, a zoonotic disease caused by the protozoan Toxoplasma gondii, poses significant public health challenges globally due to its high seroprevalence and varied clinical manifestations. As an obligate intracellular parasite, T. gondii can infect all warm-blooded vertebrates, but felids are its only definitive hosts, shedding unsporulated oocysts into the environment. Humans typically acquire the infection through ingestion of tissue cysts in undercooked meat or oocysts from...

Sputum Studies II: Culture and Sensitivity

Description
Sputum culture and sensitivity is a medical procedure used to diagnose bacterial infections in the respiratory tract and select the most appropriate antibiotics for treatment. This process involves analyzing sputum samples of thick and opaque secretions produced in the lungs and airways. These samples are collected from patients and then sent to the laboratory for analysis.
The test can identify various pathogens responsible for respiratory infections, including Streptococcus,...

Summary

Most patients with positive Strongyloides serology were asymptomatic. Early treatment before immunosuppression is recommended, but current regimens lack uniformity, indicating a need for standardized protocols for Strongyloides infection management.

Area of Science:

  • Medical Parasitology
  • Infectious Diseases
  • Clinical Serology

Background:

  • Strongyloides stercoralis is an intestinal nematode with 2%-7% prevalence in the US.
  • Immunosuppression increases risk for disseminated Strongyloides infection.
  • Diagnosis and treatment are often delayed, necessitating better screening and management strategies.

Purpose of the Study:

  • To review the prevalence of positive Strongyloides antibody results.
  • To describe the clinical characteristics and outcomes of patients with positive serology.
  • To identify gaps in current management protocols.

Main Methods:

  • Retrospective review of patients with positive Strongyloides IgG serology (August 2012-April 2023).
  • Classification of clinical presentation into chronic infection, hyperinfection syndrome, and disseminated infection.
  • Analysis of patient demographics, indications for testing, and treatment administration.

Main Results:

  • 412 patients identified; median age 59, 66% male.
  • Most common indication for testing was pretransplant evaluation (56%).
  • 97.3% had chronic infection, 2.42% hyperinfection, 0.24% disseminated infection; 89% received treatment.

Conclusions:

  • The majority of patients with positive Strongyloides serology were asymptomatic.
  • Treatment prior to immunosuppression is advised, but regimens were not uniform.
  • Variability in treatment highlights the need for standardized Strongyloides management protocols.
