Abstract:
This case report outlines the multidisciplinary perioperative management of a 69-year old female with a giant ovarian borderline tumor and severe scoliosis who underwent total hysterectomy and bilateral salpingo-oophorectomy. Preoperative optimization incorporated respiratory training and nutritional support. Intraoperative interventions included video laryngoscopy, lung-protective ventilation, individualized positioning, and goal-directed fluid therapy with prophylactic vasopressor use during tumor resection. The postoperative course was complicated by upper gastrointestinal bleeding, managed endoscopically. This experience reinforces the necessity of a physiology based, nurse-coordinated multidisciplinary strategy in patients with concurrent giant pelvic tumors and spinal deformity, and advocates for gastrointestinal prophylaxis in chronic NSAID users undergoing major surgery.
