Summary

Postoperative cognitive dysfunction (POCD) affects 15.5% of patients after aortic arch replacement. Advanced age, female sex, and longer antegrade cerebral perfusion time predict POCD, which does not impact midterm survival.

Background:

Postoperative cognitive dysfunction (POCD) is a frequent complication following aortic arch replacement surgery.
Identifying risk factors and long-term outcomes of POCD is crucial for patient management.

Purpose of the Study:

To identify risk factors for early POCD in patients undergoing aortic arch replacement.
To evaluate the impact of POCD on long-term survival after this procedure.

Main Methods:

A prospective cohort study included 103 patients undergoing aortic arch replacement.
Cognitive function was assessed preoperatively and postoperatively using the Montreal Cognitive Assessment.
POCD was defined as a ≥20% decline in cognitive scores.

Main Results:

The incidence of POCD was 15.5%.
Independent risk factors for POCD included advanced age, female sex, and prolonged antegrade cerebral perfusion (ACP) time.
POCD did not significantly affect midterm survival rates.

Conclusions:

Advanced age, female sex, and prolonged ACP time are significant predictors of POCD after aortic arch replacement.
POCD does not appear to adversely affect midterm survival.
These findings aid in preoperative risk assessment and intraoperative strategy optimization.
