Background:

  • The RAPIDO trial showed short-course radiotherapy followed by chemotherapy and surgery reduced treatment failure but increased locoregional recurrence.
  • Improving local control in rectal cancer is a key challenge.
  • Intensified radiotherapy dose is a potential strategy to enhance local tumor control.

Purpose of the Study:

  • To evaluate the efficacy and safety of intensified MRI-guided radiotherapy with a simultaneous integrated boost (SIB) in locally advanced rectal cancer.
  • To assess rates of pathological complete response (pCR) and clinical complete response (cCR) with a watchful-waiting approach.
  • To determine the incidence of local and distant recurrence following this total neoadjuvant treatment (TNT) strategy.

Main Methods:

  • Retrospective analysis of 28 patients with cT3-4 N0-2 rectal cancer treated between November 2021 and January 2024.
  • Treatment involved adaptive MRI-guided radiotherapy (5 × 5 Gy on mesorectum, 5 × 6 Gy SIB on GTV) followed by 18 weeks of CAPOX or FOLFOX chemotherapy.
  • Watchful waiting was considered for patients achieving cCR; surgery was performed otherwise.

Main Results:

  • 27 patients were analyzed with a median follow-up of 39 months.
  • Pathological complete response (pCR) was achieved in 6 patients (22%) after surgery.
  • Seven patients (26%) maintained a persistent clinical complete response (cCR) under watchful waiting.
  • One locoregional failure (4%) and four distant metastases (15%) were observed; one patient died.

Conclusions:

  • Total neoadjuvant treatment (TNT) using MRI-guided radiotherapy with an integrated boost is a tolerable approach for locally advanced rectal cancer.
  • This strategy achieved high pCR and persistent cCR rates, indicating significant treatment response.
  • The observed low rates of locoregional failure and distant relapse suggest potential for improved outcomes in rectal cancer management.
