Transcutaneous Auricular Vagus Nerve Stimulation as a Potential Novel Treatment for Preoperative Anxiety: A Narrative

Purpose:
Preoperative anxiety is a prevalent psychological concern that can considerably impair patients' recovery and surgical outcomes. This emotional state is influenced by various factors, including the anticipation of surgery, fear of complications, and pain management. Traditional treatment modalities, encompassing both pharmacotherapy and non-pharmacological interventions, exhibit notable limitations, underscoring an urgent need for innovative therapeutic strategies that effectively address preoperative anxiety. This review aimed to understand the potential role of transcutaneous auricular vagus nerve stimulation (taVNS) in alleviating anxiety prior to surgical procedures.
Design:
A Narrative Literature Review METHODS: The following databases were searched: PubMed and Google Scholar. The searching terms and combinations of the following keywords were: "transcutaneous auricular vagus nerve stimulation OR taVNS OR vagus nerve stimulation" AND/OR "preoperative anxiety OR perioperative anxiety OR anxiety" within the time frame of 2000-2025.
Findings:
In recent years, transcutaneous auricular vagus nerve stimulation (taVNS) has emerged as a promising non-invasive neuromodulation technique. Our findings suggest that taVNS may provide a novel and effective intervention for reducing preoperative anxiety by influencing neurophysiological pathways associated with stress response and emotional regulation. The exploration of its efficacy and safety in clinical settings is essential, particularly given the gaps identified in current research regarding its application in diverse patient populations.
Conclusions:
This article highlights the need for further empirical studies to elucidate the efficacy and safety profile of transcutaneous auricular vagus nerve stimulation in clinical practice. Addressing the challenges associated with its integration into standard treatment protocols may ultimately yield more effective therapeutic options for patients suffering from preoperative anxiety, enhancing their overall surgical experience and recovery outcomes. Future research endeavors should focus on long-term outcomes and the optimization of taVNS application for maximum therapeutic benefit.
