The lower eyelid is a high-incidence site for eyelid malignancies. Wide tumor excision often results in varying degrees of full-thickness eyelid defects. Larger defects require anterior lamella reconstruction with flaps and posterior lamella reconstruction with conjunctival flaps or autologous mucosal grafts. These are often associated with donor-site complications or the need for a second-stage procedure. This article reports a case of a full-thickness lower eyelid defect involving one-half of the total eyelid length, which was successfully reconstructed using a local bipedicle advancement conjunctival flap for the posterior lamella lining, combined with a lateral cheek rotation flap for anterior lamella soft tissue coverage. This technique achieved functional and aesthetic eyelid reconstruction. Follow-up results showed good lower eyelid appearance and complete eyelid closure, with stable and reliable long-term outcomes. Moreover, this technique is simple and easy to perform, providing a reliable option for eyelid reconstruction following lower eyelid tumor resection.
