Clinical outcomes and changes in higher-order aberrations following ray-tracing-guided LASIK: A retrospective study

Purpose:
To validate the visual, refractive and higher-order aberration outcomes of a simplified ray-tracing-guided LASIK adjustment strategy in a large real-world cohort.
Methods:
This retrospective single-arm study included 117 consecutive patients (234 eyes) who underwent ray‑tracing‑guided LASIK between December 2024 and December 2025. Preoperative and 3 months postoperative examinations included uncorrected distance visual acuity (UDVA), manifest refraction, and higher‑order aberrations (HOAs) measured with an iTrace aberrometer.
Results:
At 3 months postoperatively, all patients achieved postoperative UDVA of 20/20 or better, with 91.0% achieving 20/16 or better. Vector analysis showed excellent astigmatism correction accuracy. Total HOAs, spherical aberration, and coma showed no statistically significant differences compared with preoperative values, whereas trefoil aberration was associated with a significant postoperative decrease. Multiple linear regression revealed that higher postoperative spherical aberration and trefoil were associated with worse UDVA.
Conclusions:
This simplified ray-tracing-guided LASIK adjustment strategy yields reliable refractive results and a decrease in trefoil aberration.
