Recurrent dislocation following total knee arthroplasty managed with a hinged revision prosthesis

Postoperative instability following total knee arthroplasty (TKA) is a serious complication that challenges both the surgeon and the patient. Management becomes particularly complex when instability is recurrent, as restoration of mechanical alignment and soft-tissue balance is critical for a successful outcome. We report a case of a man in his sixties who presented with knee dislocation after a primary TKA performed elsewhere. The patient subsequently underwent tibial insert exchange, following which he again experienced recurrent dislocation. Definitive management was achieved with revision TKA using a rotating hinge knee prosthesis. This case underscores the importance of appropriate implant selection and the role of constrained designs in managing complex and recurrent instability following TKA.
