Integrative modelling of interindividual cognitive variances related to successful cochlear implantation: a cohort

Objective:
To investigate the influence of cognitive factors on cochlear implant (CI) outcomes in post-lingually hearing-impaired adults, using a predictive model based on pre-implantation characteristics.
Design:
Cumulative Link Mixed Models (CLMM) were developed to test the influence of cognitive factors such as memory (verbal and visual) and fluency on speech perception in noise across multiple postoperative time points.
Study Sample:
84 patients (18-87 years) with bilateral severe to profound hearing loss were recruited during pre-cochlear implant assessment at a tertiary academic medical centre. Participants were stratified into poorer, average and better performers.
Results:
Better preoperative verbal memory predicted superior speech perception-in-noise (SIN) performance. Congenital and progressive congenital hearing loss were associated with poorer SIN outcomes. Age and duration of hearing loss did not significantly predict CI outcomes. Longitudinal follow-up demonstrated ongoing improvement in speech-in-noise perception over time.
Conclusion:
Preoperative cognitive assessment enhances prediction of CI outcomes. Early identification of Poorer Performers enables personalised, mechanism-based intervention strategies that may maximise CI benefits and improve long-term functional outcomes.
